Our Practice

Orthopaedic Care of Long Island, P.C. provides general orthopedic care for adults and children with an emphasis on adult reconstructive surgery (total joint replacements and revision surgery). In addition, we treat most sports related injuries including fracture care, arthroscopy of the knees and shoulders, and the treatment of sprains and strains. We also provide our patients with physical therapy services. We also have, as part of our facility, a Hitachi Open MRI which is certified by the American College of Radiology.

In order for our doctors to provide a complete and comprehensive medical opinion it is important that they review all your medical information. Please be prepared to provide this information to our office along with your current medical insurance information. Below is a list of new patient forms, no fault patient forms and workers' compensation forms. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
